If you are a new patient, you should download the 3 forms below before coming to see Dr. Buchfuhrer in his office. Please fill out all three forms fully and bring them with you to our office on your first visit. If you have any problems filling out these forms, please call our office for help.

Please note:

New patients should also bring in all their current medications. Please bring the actual pill bottles, inhalers, tubes (for creams) or bottles (for drops) as lists may be inaccurate or different than your actual medications.

Patient coming in for pulmonary consultations (for lung problems) should bring their last chest x-ray and any old ones that are available for comparison. CT scans of the chest should also be brought in if available.  In addition to the actual chest x-ray films (or CD ROM if the x-rays are digital), please bring in a copy of the x-ray reports.

Patients coming in for sleep consultations should bring in copies of all their old sleep studies (if available).
